2025 Fantasy Outlook
Alcantara spent the year rehabbing from Tommy John surgery undergone in October 2023. The timing of the procedure limited the right-hander to bullpen sessions, though he began to face live hitters in mid-September. All the progress reports were encouraging, but when he takes the hill in the Grapefruit League, it will be more than16 months since Alcantara threw a ball in anger. Typically, the exercises involved in Tommy John rehab strengthen the area, allowing the pitcher to match, and often add, velocity. The concern is always command and control, which were Alcantara's main assets before the injury. Even with a 98-mph fastball, Alcantara's strikeout rate was above average, but not elite. Takes loss in quality start
Alcantara (2-2) took the loss Wednesday, giving up three runs on five hits and two walks over six innings as the Marlins were downed 5-2 by the Reds. He struck out four.

The quality start was the first of the season for Alcantara, who needed 102 pitches (68 strikes) to cover six innings for the first time in 2025. The 29-year-old righty is still trying to regain his form after Tommy John surgery, and he'll carry a 6.56 ERA, 1.33 WHIP and 17:12 K:BB through 23.1 innings into his next outing, which is scheduled to come on the road early next week against the Dodgers.

The consensus throughout the industry has been that Alcantara will be traded ahead of the July 31 deadline, and while that might very well happen, Sammon hears no definitive call has been made on that front. Alcantara is under team control at a relatively modest rate through 2027, so Miami could conceivably wait to trade him at a later date. The club also surely wants Alcantara to increase his trade appeal by pitching better, as he's posted a 4.70 ERA and just a 12:8 K:BB over 15.1 frames across his first three starts.
